Integrated Behavioral Medicine

At BHB we believe there is no health without your mental health. Particularly, individuals with mental illness are at an increased risk of physical health problems compared with the general population. Our Integrated Behavioral Medicine team provides patient-centered care for high risk/vulnerable populations struggling with medical and behavioral health. The treatment team provides care that is holistic in every aspect through care integration with special attention to both the physical and mental needs of the population served.

Medical and Behavioral Health Care

A medical assessment is performed to determine clinical appropriateness, along with a no-cost psychiatric evaluation to determine the most effective treatment plan and course. We provide a no-cost medical assessment in-person or via telehealth technology at various facilities to include but not limited to Nursing Homes, Assisted Living Facilities, Rehabilitative Care Facilities, hospitals, clinics, surgical centers, and other healthcare settings. The assessment is completed by a licensed RN to determine the appropriate level of care and the totality of physical and behavioral health needs for the patient.

We have a team of dedicated medical and psychiatric physicians available around-the-clock for urgent and routine clinical needs. The Integrated Behavioral Medicine team provides each patient with an individualized treatment plan, and frequent reassessment to ensure optimal care, healing, and recovery. The goal of the program is to provide patients that have varying medical complexities with psychiatric inpatient crisis stabilization and/or chemical dependency treatment.

Integrated Behavioral Medicine Treatment Examples

Our Integrated Behavioral Medicine team is able to accommodate and treat several medical comorbidities. Some examples include the following:

  • Wound Care needs (Wound Care certified RN onsite) up to a stage 3 (stage 4 on a case-by-case bases).
  • IV Hydration & Antibiotic Therapy
  • Diabetes Type I & II
  • Stroke & other neurological Deficit
  • Catheterization – Foley/Suprapubic
  • COVID-19 + patients (screening, isolation, testing, treatment, discharge planning)
  • Complicated Urinary Tract Infections
  • Ostomy Care

The Integrated Behavioral Medicine team will assure that your patient receives comprehensive care while undergoing therapeutic treatment. The team will assure that each patient will receive the following individualized services:

  • Collaboration with referring professional by clinical team at admissions and discharge.
  • Patient and family education regarding self-care and prevention
  • Interdisciplinary team to discuss co-morbidities
  • Case management and social work services
